Vorana, Last to Fall is one of those bosses. She is tied exclusively to "The Grand Expedition" questline, meaning you won't encounter her by pure luck.
To face her, you need to unlock her specific logbook, map out her island node on the Atlas, and manually blow open her tomb using Expedition explosives. Here is a practical, step-by-step breakdown of how to track her down and survive the encounter.

With the Black Scythe Islands now visible on your Atlas, you need to scan the nodes carefully to find her exact hiding spot.
Once you load into the map, you aren't quite in the arena yet. You still have to play the classic Expedition mechanic to unlock her underground lair.

- Head over to the Ruins of Kingsmarch and talk to Dannig and Makoru to pick up Makoru's Logbook.
- Open the logbook on your Atlas, navigate through the revealed nodes, and hunt down the first boss of the chain: Medved, the Fallen Seer.
- Once you defeat Medved, pay close attention to the loot on the ground—he will directly drop Vorana's Logbook. Grab it, as this is your golden ticket to her domain.
Now that you have Vorana's Logbook in your inventory, it's time to chart the path to her location.
- Take the logbook back to Kingsmarch and open it right on your Atlas. This action uncovers a unique network of Ocean biome maps known as the Black Scythe Islands.
- Pro-Tip: If you happen to have a Vorana's Saga Omen, make sure to keep it active in your inventory. Doing this guarantees the correct boss path structure layout, saving you unnecessary guesswork.

- Look through the newly revealed islands for a specific node titled "The Last to Fall...". It usually features a Mournful Cliffside layout.
- To confirm you have the right place, look for a distinct red boss icon overlaid on top of that island node.
- Keep in mind that you cannot just run this on any low-tier setup. You will need to insert a Tier 11 or higher Waystone into your device to activate and enter this map zone.
Once you load into the map, you aren't quite in the arena yet. You still have to play the classic Expedition mechanic to unlock her underground lair.
- Clear out the initial packs of monsters in the area and locate the NPC Tujen.
- Scout around the zone until you find the structural entrance marked as Vorana's Tomb.
- Set up your chain of Expedition explosives carefully. You must ensure that the blast radius directly overlaps and detonates the Tomb structure itself.
- Once the explosion triggers, a path will open up. Head down into the newly revealed underground lair to finally begin the encounter.
- Stay inside the safe circle: The absolute most important mechanic to know beforehand is the Dark Fog. As soon as Vorana spawns, she blankets the entire arena in a lethal mist. You must stay inside the tiny, moving safe circle directly surrounding her. If you step outside into the fog, the damage-over-time is insanely high and will melt your character within 1 to 2 seconds.
- Circle strafe constantly: Because the safe circle forces you into tight, close-quarters combat, you cannot stand still. Constantly run in circles around her to reposition behind her back. This makes it incredibly easy to dodge her heavy forward-facing Double Scythe Slashes and Lacerate shockwaves.
- The Rewards: Defeating Vorana is well worth the effort. She drops Uthred's Logbook, which is essential for progressing to the next stage of the Grand Expedition questline, along with high-tier Runes of Aldur to boost your gear.
